If you are not a current CMU student (enrolled within the last year), complete the CMU application before submitting your program application. Please note, acceptance to CMU does not guarantee acceptance into the nursing program.

There are TWO separate applications for the PN (Practical Nursing) program: one for the Montrose cohort, and one for the Grand Junction cohort. Please fill out and submit only ONE application form for the cohort you are interested in. Applicants will only be considered for ONE cohort.

BSN Application Form
This application is for the traditional BSN (tBSN) program. This is NOT the application for individuals who already possess an LPN who are seeking admission to the LPN-BSN program nor an individual who possess an ASN/AASN RN seeking admission to the online RN-BSN program. This application must be DOWNLOADED prior to filling it out.

This program is for those who already have their RN license and want to go on to earn a BSN degree.

Kaplan Nursing Entrance Exam
Kaplan testing is not required for the LPN-BSN, or the RN-BSN Program.
The Kaplan Nursing School Entrance Exam is available to take throughout the academic year.
Results of the Kaplan Exam are required to be turned into the Department of Health Sciences by the application deadline. Students are allowed to take the KAPLAN only ONE time per application cycle (either fall or spring).
If the Kaplan Nursing School Entrance Exam is completed at the CMU Testing Center, the Department of Health Sciences' will pull the results directly. For candidates taking the Kaplan Nursing School Entrance Exam at another Testing Center, it is the candidate's responsibility to work with the CMU Testing Center to set up the exam so that CMU has access to the results.
