The University has invested in a survey software, Qualtrics, to help facilitate survey research. To ensure the university upholds high research standards and integrity, faculty, staff and students who wish to use Qualtrics to conduct surveys must complete Collaborative Institutional Training Initiative (CITI) training. Some survey projects will also need to be reviewed by the Institutional Review Board (IRB).
Please read all the items below to determine what your next steps should be.
All survey creators/administrators must do the following:
- Read the Colorado Mesa University survey policy
- Pass the appropriate CITI training course from the list below. Read this guide for directions on selecting the correct CITI course
- Social and Behavioral Science Research Course (ID 10089) or
- Biomedical Research (ID 10088)
- Complete the Qualtrics Account Request. You will need your CITI number to complete the form.
Survey creators/administrators may need to do the following:
- Determine if the survey requires review by the IRB and, if so, submit a research protocol to the IRB. Contact the Office of Sponsored Programs (OSP) for more information.
- If more than 50 campus constituents will be surveyed, IRPDS must be consulted to ensure a university wide survey is not being administered at the same time.
- If the survey is a class project and the researcher intends to distribute the survey via email to campus constituents outside of that class, IRPDS must be consulted prior to distribution.

After you have completed the Qualtrics Account Request, allow two business days for your Qualtrics account to become available. You will be contacted once it is available.
If additional survey research is done in the future, the above guidelines must be followed and IRPDS and/or the OSP must be consulted if necessary.
